Sample Caregiver Contract (California)

Parties Involved:

Caregiver: Jane Doe
Address: 123 Maple Avenue, Los Angeles, CA 90001

Client/Recipient: John Smith
Address: 456 Oak Street, Los Angeles, CA 90002

Services:

The caregiver agrees to provide assistance with daily living activities, companionship, and other agreed-upon support services at the client’s residence located at the address above, or as otherwise specified.

Compensation:

The client shall pay the caregiver a rate of $20 per hour, payable weekly or bi-weekly, as mutually agreed. Overtime, if applicable, shall be compensated at 1.5 times the regular rate after 40 hours per week.

Caregiver Responsibilities:

The caregiver shall perform duties reliably, maintain confidentiality, adhere to all applicable laws and regulations, and uphold professional standards of care.

Term and Termination:
